You must agree to these Terms of Service to Use any CNHSA.com Service or Product.

By using or registering for any service or product of cnhsa.com you agree to these terms and conditions (“Terms of Service”). Products and services of cnhsa.com refer to any information or programs viewed or obtained on any cnhsa.com, choctawnationhealth.com, or any authorized web based platform controlled or owned by Choctaw Nation Health Services Authority(cnhsa).

No Spam, Bulk Email, or Bulk Messages

You may not use any product or service including messaging services for patients or patient advocates to deliver or send unsolicited bulk communications. You may not harvest or collect information using any cnhsa.com service or product. You agree that Choctaw Nation Health Services authority may take any legal or technical remedies to prevent unauthorized or misappropriate communications on our networks.

No Inappropriate, Illegal, or Unsolicited messages.

No service or product of cnhsa.com may be used to communicate messages that may be deemed Inappropriate. Inappropriate messages meaning messages which have content such as:
1) Offensive language.
2) URLs (links) to offensive content such as porn websites or “rotten” websites.
3) Threats or messages intending anything that could be construed as harmful.
4) Harmful or unsolicited Code such as spam or viruses.
5) Unsolicited Links or advertisements that could be construed as spam.
6) Distasteful jokes.
7) Illegal or malicious information.

Monitoring

You consent to be monitored when a service or product of cnhsa.com is used. Whenever any system is submitted a log will be kept including timestamp and identification information obtained from your machine. This is kept only as a means to track any TOS violations and you understand this will not be used in any way other than this.

Liability

You agree that Choctaw Nation Health Services Authority will not be held liable for any damage or corruption of data on your machine. CNHSA and its vendors shall not be liable for any indirect, special, incidental, consequential or exemplary damage arising from your use of any cnhsa.com product or service. These exclusions for indirect, special, incidental, consequential or exemplary damages include, without limitation, damages for lost profits, lost data, loss of goodwill, work stoppage, computer failure or malfunction, or any other commercial damages or losses, even if CNHSA had been advised of the possibility thereof and regardless of the legal or equitable theory upon which the claim is based. CNHSA does not endorse, warrant, or guarantee any product or service offered through cnhsa.com and will not be a party to or in any way be responsible for monitor any transaction between you and third-party providers of products or service. CNHSA will not be held liable for the content of any program or service to the extent mentioned above.
