Eligibility
- Must reside within the Choctaw Nation service area for any referral services
- Must be a tribal member of the Choctaw Nation
- Patient must be 18 years of age or older
- Patients between the ages of 2-18 may be seen in Talihina
How to Apply
Requires a referral from a primary care provider.
